Procedure for applying for a medical practice certificate

1. After receiving the application for issuance or re-issuance of a practice certificate (CCHN), the Ministry of Health or the Department of Health (hereinafter referred to as the application receiving agency) shall issue to the applicant a Application Receipt according to the form prescribed in Appendix II issued with Decree 109/2016/ND-CP specifically as follows:

Submit application directly to the receiving agency: if the application is complete according to regulations, the receiving agency will send it immediately. Application receipt for applicants for issuance or re-issuance of practice certificates.
Sending documents by post: within 02 days from the date of receiving the documents (based on the postmark), the agency receiving the documents will send Application receipt for applicants for issuance or re-issuance of practice certificates.

2. Within 20 days from the date stated above Application receiptThe agency receiving the application must conduct an appraisal of the application and have an appraisal record.

3. In case of invalid documents:

Within 05 working days from the date of the appraisal report, the agency receiving the dossier must issue a written notice to the applicant for issuance or re-issuance of the Certificate of Business Registration to complete the dossier. The written notice must specifically state the documents that need to be supplemented and the content that needs to be revised.
Upon receiving a written request to complete the application, the applicant for issuance or re-issuance of a Certificate of Business Registration must amend and supplement the application in accordance with the contents stated in the document and send the supplementary application to the application receiving agency. The application receiving agency shall send a Receipt of Supplemental Application to the applicant for issuance or re-issuance of a Certificate of Business Registration.
The agency receiving the application shall continue to carry out the procedures according to regulations. In case the supplementary application is still invalid, the agency receiving the application shall continue to guide the applicant to complete the application.
In case the dossier receiving agency has requested to amend or supplement the dossier, but within 60 days from the date the dossier receiving agency has a written request, the applicant for issuance or re-issuance of the Certificate of Business Registration does not supplement or amend the dossier, the application must be redone from the beginning.
